While we will gladly submit claims for care rendered in our office, it is important for patients to understand that final responsibility for collection of benefits from your insurance company rests with the insured party.
There are many different agreements between carriers and their subscribers. Each contract provides a different benefit. Unlike ordinary dental insurance, orthodontic insurance usually provides each insured person with a lifetime maximum benefit for orthodontic services. This benefit is often paid as a percentage of the orthodontic fee until the maximum is reached.
We will reach out to your insurer to determine coverage, and out of pocket costs. Simply provide us with the name, address, telephone number of your insurance carrier, the subscriber ID number, and the company group number. As the responsible party, you should also verify the treatment estimate you receive from us with your insurance company.
- We will accept payments from your insurer assigned to us by the insured. Such payments will be credited accordingly to your account.
- If your company requires verification of continuing treatment, you may send them copies of your cancelled checks or cash receipts.
- If your insurance carrier provides a form for verification of continuing treatment, we will gladly complete the signature portion.
